Educational Goals, Objectives, and Policies

For Residents in the Department of Anesthesia

At the Indiana University School of Medicine

Educational Goals and Objectives (Global)

The goal of the Department of Anesthesia is to provide a sound education that prepares residents to become qualified practitioners of anesthesia at the superior level of performance expected of a board certified consultant anesthesiologist. To this end, we have an expectation that all residents who complete their Anesthesia training in the Indiana University program will become board certified and will provide high quality competent patient care. We anticipate that they will strive to improve the practice of anesthesia at the local, state and national level.

The ACGME defines Competencies as: Specific knowledge, skills, behaviors and attitudes and the appropriate education experience required of resident to complete Graduate Medical Educational (GME) programs. It seems logical that the natural progression of competence is a progressive movement from incompetence to competence followed by mastery of knowledge, skills, behaviors and attitudes. One of the primary goals of the Department of Anesthesia is to train anesthesiologist that exceed the expectation of competence and move towards and reach for overall mastery of the above mentioned set of objectives.

We believe that the competencies that are outlined below by the Accreditation Council for Graduate Medical Education (ACGME) provide a template that we can use to help progress our residents through the various stages of their educational experience towards our goal of mastery.

PATIENT CARE

Residents must be able to provide compassionate, appropriate, and effective patient care for the diagnosis and treatment of health problems and for promotion of health. Residents are expected to:

communicate effectively and demonstrate caring and respectful behaviors when interacting with patients and their families

gather appropriate and accurate information about their patients

make informed decisions about diagnostic and therapeutic interventions based on patient information and preferences, up-to-date scientific evidence, and clinical judgment

develop and carry out patient management plans

counsel and educate patients and their families

use information technology effectively to support patient care decisions and patient education

perform competently all essential diagnostic and therapeutic procedures for the practice of anesthesia

provide health care services aimed at preventing health problems and maintaining health

work with health care professionals, including those from other disciplines, to provide patient-focused care

MEDICAL KNOWLEDGE

Residents must demonstrate knowledge about established and evolving biomedical, clinical, and cognate (e.g. epidemiological and social-behavioral) sciences and the application of this knowledge to patient care. Residents are expected to:

demonstrate an investigatory and analytic thinking approach to clinical situations

know and apply the basic and clinically supportive sciences that are appropriate to anesthesiology

INTERPERSONAL AND COMMUNICATION SKILLS

Residents must be able to demonstrate interpersonal and communication skills that result in effective information exchange and collaboration with patients, their patients families, and professional associates. Residents are expected to:

create and sustain a therapeutic and ethically sound relationship with patients

use effective listening skills and elicit and provide information using effective nonverbal, explanatory, questioning, and writing skills

work effectively with others as a member or leader of a health care team or other professional group

PROFESSIONALISM

Residents must commit to carrying out their professional responsibilities, adhere to ethical principles, and be sensitive to a diverse patient population. Residents are expected to:

demonstrate respect, compassion, and integrity; a responsiveness to the needs of patients and society that supersedes self-interest; accountability to patients, society, and the profession; and a commitment to excellence and on-going professional development

demonstrate a commitment to ethical principles pertaining to provision or withholding of clinical care, confidentiality of patient information, informed consent, and business practices

demonstrate sensitivity and responsiveness to patients’ culture, age, gender, and disabilities

SYSTEMS-BASED PRACTICE

Residents must demonstrate an awareness of and responsiveness to the larger context and system of health care. Residents must have the ability to effectively call on system resources to provide care that is of optimal value. Residents are expected to:

understand how their patient care and other professional practices affect other health care professionals, the health care organization, and the larger society and how these elements of the system affect their own practice

know how types of medical practice and delivery systems differ from one another, including methods of controlling health care costs and allocating resources

practice cost-effective health care and resource allocation that does not compromise quality of care

advocate for quality patient care and assist patients in dealing with system complexities

know how to partner with health care managers and health care providers to assess, coordinate, and improve health care and know how these activities can affect system performance

PRACTICE-BASED LEARNING AND IMPROVEMENT

Residents must be able to investigate and evaluate their patient care practices, appraise and assimilate scientific evidence, and improve their patient care practices. Residents are expected to:

analyze practice experience and perform practice-based improvement activities using a systematic methodology

locate, appraise, and assimilate evidence from scientific studies related to their patients’ health problems

obtain and use information about their own population of patients and the larger population from which their patients are drawn

apply knowledge of study designs and statistical methods to the appraisal of clinical studies and other information on diagnostic and therapeutic effectiveness

use information technology to manage information, access on-line medical information; and support their own education

facilitate the learning of students and other health care professionals

GOALS and OBJECTIVES:

The goal of the Peri-operative Care Unit (POCU) Rotation at the Indiana University School of Medicine is to train physicians to be competent and compassionate physicians when caring for patients in the POCU. This rotation is designed to develop the appropriate knowledge, understanding and application of anesthesia principle in order to provide safe management and appropriate evaluation of the POCU patient.

Basic POCU Resident Duties:

Basic duties for the PACU/POCU residents include:

The resident, during the PACU/POCU rotation, will be available for duty from 0700 – 1600 hrs.

During the POCU rotation the resident will learn to evaluate and develop an anesthetic plan for the administration of a safe general, regional anesthetic or monitored anesthesia care

Attend didactic conferences

Take night call on a rotating schedule

Respond to requests for anesthesia consults in a timely fashion

Maintain a personal program of self-study and professional growth

Complete medical records promptly

Document all duty hours

Document all procedures

Monitor self for fatigue

Dress appropriately

Act in a professional and ethical manner

Complete the appropriate evaluation instruments used by the department, including QA forms

Educational Strategy:

The POCU Rotation consists of two, two week rotations provided during either the second half of your CA1 or CA2 years of training for rotation designated POCU/PACU or during the CA2 or CA3 years for the rotation designated POCU/UOR. During this period, the resident will be involved in direct patient care with the attending staff. When indicated, immediate patient care needs will be discussed with the attending physician in charge of the overall anesthetic management for the individual patient. Dr. Robert Byers, the Medical Director of the POCU/PACU and is responsible for the administrative and educational aspects of the POCU/PACU rotation and will provide residents with a selected group of review and journal articles. Dr. Mark Tasch will serve as the POCU/PACU director in Dr. Byer’s absence. The resident on this rotation while in the POCU will provide the attending faculty assigned to this patients anesthetic or the on-call faculty with an overview of all ongoing patient care issues in the POCU as needed. During the rotation residents are expected to participate in journal clubs and morbidity and mortality conferences (QA).

On July 1, 2008 the ACGME instituted a change in the total duration of the rotations required in the POCU. Subsequently a POCU/UOR rotation has been instituted. Two residents will be assigned to this rotation during any four week rotation block. One resident will begin on the POCU rotation and the other resident will be assigned to the OR for two week periods. As part of this rotation, the resident on POCU will be also be responsible for providing coverage for the peripheral nerve block rotation at University. The UOR resident on the POCU/UOR rotation will typically be assigned to the operating room in which patients undergoing peripheral nerve blocks by the POCU resident are performed. Since this rotation consists of two, two week rotations each resident will effectively work as a team to support the other resident’s experience. Dr. John Wolfe will be the Medical Director of the POCU/UOR rotation and is responsible for the administrative and educational aspects of the POCU/UOR rotation and will provide residents with a selected review and journal articles. Dr’s Yar Yeap and Kevin Stockmaster will serve as the POCU/UOR director in Dr. Wolfe’s absence. The resident on this rotation while in the POCU assist the POCU/PACU resident and will provide the attending faculty assigned to this patients anesthetic or the on-call faculty with an overview of all ongoing patient care issues in the POCU as needed.

For a more comprehensive understanding of the responsibilities associated with the expectations associated with the peripheral nerve block portion of this rotation, residents should refer to the goals and objectives associated with the peripheral nerve block rotation.

Core Competencies:

The six core competencies are used as a template to evaluate residents during all Anesthesia rotations. The terms used to define these competencies are similar to those used for other rotations. The Goals and Objectives that follow have been specifically modified to meet the needs of the specific teaching rotation in which you are to participate. These Goals and Objectives are not intended to be comprehensive but have been developed to help you acquire the core competencies in the area of Clinical Anesthesia. These core rotations should serve as the foundation upon which the subspecialty anesthesia rotations are based. We ask you as the learner to consider how each of the subcategories within these six competencies might relate to cognitive, motor and affective characteristics of your education and your professional behavior. If while reading this document you discover an area that requires revision or improvement please bring these issues to the attention of either the Course Director or the Program Director. We seek changes that will improve your educational experience.
